Noisy manhole cover, possible danger to cyclists

A section of the Upper Richmond Road West was recently resurfaced and since then a manhole cover has become dislodged or loosened and as such, is very noisy every time a car goes over it. This has become a bad distraction and an irritant, trying to sleep at night. I also fear that if the cover is loose it may cause a serious injury to a bicycle or motorcycle.
