Over the past few weeks, the pedestrian light sometimes self-cancels without changing the traffic lights to red and people can be caught out and assume that it is safe to cross when it is not. This morning at around 09.10 this was the cause of a nasty accident with a dog

As above, when the pedestrian white "wait" light goes out spontaneously, without triggering a cross signal or red traffic light, people either have to start again, and re-press, if they notice, or, as in this morning's accident, especially if the traffic in one direction is stopped already because of congestion, assume it is safe to cross, and can get hit by traffic in the further lane.
