Loose Manhole Cover

The problem has been ongoing for months. Every time someone tries to fix the cover, it is ok for a week or so, then it loosens again and reverts to noisy 'CLUNK CLUNK!' sounds every minute or so. Please look in to this as soon as possible as this is directly outside our home and myself and the residents are finding it unbearable.
