Very dangerous loose man hole cover

Dear Sirs
I would like to report a very dangerous loose man hole cover on the bus lane outside 54 Stoke Newington Road (the A10) road in Hackney. It is situated in the bus lane and every time a bus or lorry goes over it it makes a very loud crashing noise.
This is obviously dangerous to road users and pedestrians and it is also keeping me and my family awake at night.
Can someone please arrange for it to be inspected and repaired ASAP?

Thank you for taking the time to report this to us. It is much appreciated. This has been investigated and we have now referred it to our contractors. If it is confirmed as a safety issue it will be made safe within two hours. If the manhole cover is owned by us, we will carry out a permanent repair at a later date. If it is owned by another company this will be passed to them for a permanent repair.
