Loose and banging gully grid on south side of west bound A40, 3rd gully West of Bloemfontein Road, W12.

Reported via desktop in the Drain Cover - Missing or Damaged category anonymously at 12:41, Mon 15 November 2021

Sent to TfL less than a minute later. TfL ref: FMS3124529.

The carriageway gully grid at this location is loose and moves within the frame when trafficked, resulting in a very loud and disturbing noise. This happens continually throughout the day and night. Not only is this a danger to highway users but also an environmental nuisance to residents in the area. This issue has been detectable for some months, first noticed the noise in September 2020 but couldn't identify where it was coming from. Finally identified the cause 12/11/2021. The gully in questions is the third gully west from Bloemfontein Road. Gully is accessible from the footway but will need to take out lane one to repair.
