Manhole cover loose and noisy then bus/truck go over it

Sent to TfL less than a minute later. TfL ref: FMS3280649.

The manhole cover is cracked in two down the middle, lose and very noisy when large vehicles go over it. The manhole was repaired by Tfl less than 6 months ago when ( it was sunken in and had huge holes around it) but it’s already broken again, and the round cover is cracked into two pieces which is very dangerous as either or both pieces could soon fall into the whole and leave the manhole completely exposed. Please fix urgently for health and safety reasons.
