Issue with cycle path

Location - cycle path Clapham High street next to Hier Generation.

Issue - There is no clear cause but the surface in circled area of the attached photo has caused at least 4 (probably more) bike accidents in the past 6 weeks, All of 4 accidents seen did not involve any other vehicle but were caused by the road surface.

I was one of 4 cyclists and fractured my wrist and shoulder and I have heard that the others also had injuries. As this is on a downhill slope cyclists will naturally be going faster making it extremely dangerous.

Length of time of issue: It is unclear how long it has been there but I would estimate 2 months or more.
