No Right turn indicator for turning right from Vivian Avenue to Hendon Way South

I am not exactly sure what is going on here. There was a working set of traffic lights until your team upgraded these lights and took away the right turn indicator. Last week, a right turn light was re-introduced but again this afternoon there was no right-turn light causing for a dangerous situation with cars stuck in the middle of the intersection and the straight lights for 6 lanes turning green at the same time. The missing right-turn light is causing severe tailbacks on Vivian Avenue and Queen's road for both right turns into Hendon Way South and North. This has got to be fixed urgently before there is a major accident in this location!
