Clanging Manhole cover

This is causing loud clanging everytime a vehicle drives over it! I see it has already been reported but nothing has been done - this could cause a serious accident as the cover is phisically liftng up and could easiliy dislodge. It is also causing unacceptable noise pollution for surrounding residents.
