Lights change after 15 seconds

These lights on the A406 North Circular road Southbound on to Chiswick roundabout are letting just 3 cars through per change. The result is standstill traffic for 3 miles, all other entries and exits from Chiswick roundabout are clear and this has been going on daily for the last 2 weeks. The phase of these lights needs to be addressed. Please see current traffic flow attached. This is not the first time this has been raised. Thank you.
