Traffic signals failure (all out)

Sent to TfL less than a minute later. TfL ref: FMS6153690.

On the 23rd June I approached this 4-way signalised junction and realised all 4 approaches had their signals not working, this could (or may already have) caused head to head collisions, it is extremely dangerous and TfL needs to check why these lights constantly go out as it had been reported and supposedly fixed in 2021. Lives are at risk, please treat as emergency.
