Manhole cover broken

Sent to TfL less than a minute later. TfL ref: FMS6418100.

Manhole cover in lane 1 is broken/loose and making a lot of noise when cars and lorries go over it. this problem has been reported many time but a permanent solution has yet to be found. workman come and fix it temporarily and then it happens again
