Manhole cover is rocking and jumping out of its enclosure

Sent to TfL less than a minute later. TfL ref: FMS7081076.
The manhole cover is rocking and jumping out of its enclosure whenever a large and heavy lorry goes over it. The surrounding area of the manhole cover has actually sunk and it is very dangerous as a vehicle can easily sink in to it and crash! This has been repaired several times, but it seems that the repair is not adequate and keeps reoccurring.

If it is confirmed as a safety issue it will be made safe within two hours. If the manhole cover is owned by us, we will carry out a permanent repair at a later date. If it is owned by another company this will be passed to them for a permanent repair. The problem of the man hole cover is urgent as the drain cover has sunk in to the road and at any time now the entire cover can disappear in to the hole and there is going to be a huge accident! This is a danger to life type scenario. Please fix this permanently as it seems to happen every coupe of months....
