Sent to TfL less than a minute later. TfL ref: FMS7101331.

A4 Cromwell road Eastbound at the junction of Lexham Gardens- so just outside the Cromwell Hospital- There is a missing man-hole cover which you have coned off, but you did that 3 months ago!! Have you forgotten about this?? How long does it take to replace a man-hole cover?? In effect, you have left the A4 down to one lane for months! Please attend to this asap.
