Dangerously Loose Pavings

One of the tactile paving on the pedestrian crossing outside Leon on Tooley Street (More London) is dangerously loose, and I tripped on it and fell on my face as I passed by on the evening of Saturday 13th September at approximately 18:27. (My mobile phone also went flying out of my hand, and was lucky not to have cracked as it had a protective cover.) It has been like this for a while now, and needs to be fixed ASAP before someone is seriously hurt. Several of the other narrow paving slabs in the More London area are also dangerously loose and I have tripped on them in the past also (I have also witnessed other people tripping on them).
